3) How do I prepare for my first office visit?

 

After you have scheduled your initial consultation, visit our website to download the New Patient Packet forms.  Fill these out and bring them with you to the first visit.

 

You may also request the forms be mailed to your home by calling any of our three locations.

 

Your first office visit does not involve any testing, so no special preparation is needed.
